BayView Home Health

In January 2005, the St. Johns Welfare Federation introduced BayView Home Health, a home health rehabilitation program.  The organization is now in a unique position to extend its continuum of healthcare by providing services that will allow residents and clients to receive additional services in their own home with the same quality staff from BayView.  This adds to their quality of life and allows for healthcare services in a familiar setting.  While the need for the services has been present in the past, this program will take a comprehensive continuum of care approach to each individual’s medical and social service needs.

At the St. Johns Welfare Federation, we believe that the patient’s happiness and interests are of primary importance.   We consider it is our responsibility to provide nursing activities and healthcare services that promote achievement of the patient’s great capability for sustained physical, emotional, social cultural, personal and spiritual health. This program provides us the opportunity to complete that circle of care for the clients we serve. 

      This home health style continuum of care program provides the following services:
